InLight Therapy Editable Rental Forms

These are forms provided by InLight Therapy for renting units.  You can rent any combination of controller and pads at any fee that you choose.  At PTI, we typically rent 2 Port Systems for $200 per week and 6 Port Systems for $300 per week.  IF the customer commits to purchasing an InLight system within the rental period, then we credit their purchase with the first TWO weeks of rental fees towards their purchase.
